This template should be used by businesses in England and Wales when they are undertaking a review of the performance of their staff.
It is vitally important for the performance of all staff to be monitored and measured in a fair, consistent and measurable way. This template will allow the member of staff to reflect upon their own performance and for you to then conduct a review of their performance with them within a structured meeting.
Goals can then be set as to how the staff member should perform over the next 12 months and the process can then be repeated once again at which time the attainment of the goals set can be assessed.
The whole process is recorded and signed by both parties.
